About The Position

The Patient Care Technician (PCT) will work on-site at DaVita Skilled Nursing Facility (SNF) dialysis dens. Approximately 10% of all dialysis patients reside in a SNF, and transporting them for dialysis places a significant burden on patients and adds avoidable costs. DSD Services teammates will leverage DaVita's capabilities and resources to develop infrastructure and rapidly grow this new service line. Ideal teammates should have significant experience within DaVita, be energized by a start-up environment, be passionate about improving patients' lives, and have a track record of finding ways to get things done. Teammates must be comfortable working autonomously and demonstrate the ability to collaborate with cross-functional resources. Local DaVita SNF Dialysis dens are located at Northland Rehabilitation and Highland Rehabilitation in Kansas City, MO. In this role, the PCT is responsible for the complete ongoing care of each patient to ensure their dialysis treatment stays on track, working in conjunction with dialysis RNs to provide care. DSD Dialysis care is a complex but fulfilling service line, with goals of meeting and/or exceeding clinical outcomes and an expectation for multitasking to meet the needs of the service, patients, and SNF partners. PCTs will provide comprehensive patient care in an independent setting, requiring constant critical thinking and application of core skills to monitor patients before, during, and after dialysis treatment, including measuring and recording stats, patient observations, and hemodialysis machine setup.

Requirements

No On Call
Dialysis Experience Required
Must have a Current CCHT
High school diploma or equivalent
MUST BE CCHT certified
Basic computer skills and proficiency in MS Word and Outlook

Responsibilities

Take responsibility for the complete ongoing care of each patient to ensure their dialysis treatment stays on track
Work in conjunction with dialysis RNs to provide dialysis care
Spend the majority of your day in direct one-on-one patient care to provide safe, comfortable and hygienic dialysis treatment
Meet and/or exceed clinical outcomes
Multitask to meet the needs of the service, the patients and our SNF partners
Provide comprehensive patient care in an independent setting
Monitor patients before, during and after dialysis treatment including measuring and recording stats, patient observations, and hemodialysis machine setup
Commit to improving patient health through clinical goal setting and quality improvement initiatives
