About The Position

This per diem role focuses on providing dialysis care to patients in Skilled Nursing Facilities (SNFs). The goal is to leverage DaVita's capabilities to develop and grow this new service line, improving the lives of patients who require dialysis three times a week. Teammates in this role will work autonomously and collaboratively with cross-functional resources, taking responsibility for the complete ongoing care of each patient to ensure their dialysis treatment stays on track. This is a complex but fulfilling service line where meeting and/or exceeding clinical outcomes and multitasking are expectations. PCTs will work in conjunction with dialysis RNs to provide dialysis care, spending the majority of their day in direct one-on-one patient care to provide safe, comfortable, and hygienic dialysis treatment. Training will include a combination of classroom and hands-on learning through DaVita’s award-winning programs. You may also be asked to support other local SNF dens.

Requirements

High school diploma or equivalent
MUST BE CCHT certified
Basic computer skills and proficiency in MS Word and Outlook

Significant experience within DaVita

Responsibilities

Take responsibility for the complete ongoing care of each patient to ensure their dialysis treatment stays on track.
Work in conjunction with dialysis RNs to provide dialysis care.
Spend the majority of your day in direct one-on-one patient care to provide safe, comfortable and hygienic dialysis treatment.
Provide comprehensive patient care in an independent setting.
Monitor patients before, during and after dialysis treatment including measuring and recording stats, patient observations, and hemodialysis machine setup.
Support other local SNF dens as needed.
