About The Position

DaVita Kidney Care is launching a new team, DaVita Skilled Nursing Dialysis Services (DSD), to provide industry-leading clinical quality to patients on-site at skilled nursing facilities (SNF). This initiative aims to reduce the burden on patients who currently travel for dialysis and lower healthcare costs. DSD teammates will leverage DaVita's resources to build and grow this new service line, working autonomously and collaborating with cross-functional teams. The role involves direct one-on-one patient care, ensuring safe, comfortable, and hygienic dialysis treatments. PCTs will work in conjunction with dialysis RNs, managing complex care needs in an independent setting. This requires constant critical thinking, monitoring patients before, during, and after dialysis, and managing various aspects of care. Training is provided through DaVita's award-winning programs, combining classroom and hands-on learning.

Requirements

• Prior Hemo-Dialysis experience
• Valid Certified Clinical Hemodialysis Technician (CCHT) certification
• High school diploma or equivalent
• Basic computer skills and proficiency in MS Word and Outlook

Responsibilities

• Take responsibility for the complete ongoing care of each patient to ensure their dialysis treatment stays on track.
• Work in conjunction with dialysis RNs to provide dialysis care.
• Provide safe, comfortable and hygienic dialysis treatment.
• Monitor patients before, during and after dialysis treatment including measuring and recording stats, patient observations, and hemodialysis machine setup.
• Manage and track various aspects of patient care.
• Support other local SNF dens as needed.
