About The Position

DaVita Kidney Care is bringing its industry-leading clinical quality to patients on-site at skilled nursing facilities (SNF) through a new team called DaVita Skilled Nursing Dialysis Services. This new service line aims to address the significant burden on patients and avoidable costs to the healthcare system associated with transporting dialysis patients residing in SNFs to dialysis three times a week. DSD Services teammates will have access to DaVita's capabilities and resources to develop and grow this new service line. Ideal candidates will have significant experience within DaVita, be energized by a start-up environment, enjoy improving patients' lives, and have a proven track record of getting things done. Teammates must be comfortable working autonomously and collaborating with cross-functional resources across the Village. In this role, you will take responsibility for the complete ongoing care of each patient to ensure their dialysis treatment stays on track, working in conjunction with dialysis RNs. You will work 3 days a week with no required on-call, but your impact on patients' lives will be felt every day. DSD Dialysis care is a complex but fulfilling service line where meeting and/or exceeding clinical outcomes are the goals. Multitasking is an expectation to meet the needs of the service, patients, and SNF partners. You will provide comprehensive patient care in an independent setting, constantly using your mind and applying your core skills to monitor patients before, during, and after dialysis treatment, including measuring and recording stats, patient observations, and hemodialysis machine setup. Requirements

  • Healthcare experience required
  • Comfort with needles
  • High school diploma or equivalent
  • Basic computer skills and proficiency in MS Word and Outlook

Responsibilities

  • Take responsibility for the complete ongoing care of each patient to ensure their dialysis treatment stays on track.
  • Work in conjunction with dialysis RNs to provide dialysis care.
  • Provide safe, comfortable, and hygienic dialysis treatment.
  • Monitor patients before, during, and after dialysis treatment.
  • Measure and record patient stats.
  • Make patient observations.
  • Perform hemodialysis machine setup.
