About The Position

DaVita Kidney Care is expanding its services to patients in skilled nursing facilities (SNF) through a new team called DaVita Skilled Nursing Dialysis Services. This initiative aims to provide dialysis care on-site to approximately 10% of dialysis patients who reside in SNFs, reducing the burden on patients and healthcare costs. The Skilled Nursing Dialysis (DSD) Patient Care Technician (PCT) will be responsible for the complete ongoing care of each patient to ensure their dialysis treatment stays on track, working in conjunction with dialysis RNs. This role requires a high degree of autonomy, critical thinking, and the ability to multitask in a complex, fulfilling service line focused on meeting and exceeding clinical outcomes. Teammates will have access to DaVita's resources and will contribute to the growth of this new service line, ideally with prior DaVita experience and a "get stuff done" attitude.

Requirements

• High school diploma or equivalent
• MUST BE CERTIFIED HEMODIALYSIS TECHNICIAN (OCDT)
• Basic computer skills and proficiency in MS Word and Outlook

Responsibilities

• Take responsibility for the complete ongoing care of each patient to ensure their dialysis treatment stays on track.
• Work in conjunction with dialysis RNs to provide dialysis care.
• Provide safe, comfortable, and hygienic dialysis treatment, spending the majority of the day in direct one-on-one patient care.
• Monitor patients before, during, and after dialysis treatment, including measuring and recording stats, patient observations, and hemodialysis machine setup.
• Support additional surrounding sites depending on business needs.
• Commit to improving patient health through clinical goal setting and quality improvement initiatives.
