Patient Care Technician
About The Position
This role focuses on providing care for patients in Skilled Nursing Facilities (SNFs) who require dialysis. The goal is to reduce the burden on patients and healthcare systems by offering dialysis services within SNFs. Teammates will leverage DaVita's resources to develop and grow this new service line, working autonomously and collaboratively with cross-functional teams. The dialysis den is located on-site at Centennial Hills. The role involves direct one-on-one patient care to ensure safe, comfortable, and hygienic dialysis treatments. It is a complex but fulfilling service line where meeting and exceeding clinical outcomes is a primary goal, requiring multitasking to meet the needs of the service, patients, and SNF partners. PCTs will provide comprehensive patient care in an independent setting, monitoring patients before, during, and after dialysis, including measuring stats, patient observations, and hemodialysis machine setup.
Requirements
- High school diploma or equivalent
- MUST BE CCHT certified
- Basic computer skills and proficiency in MS Word and Outlook

Responsibilities
- Take responsibility for the complete ongoing care of each patient to ensure their dialysis treatment stays on track.
- Work in conjunction with dialysis RNs to provide dialysis care.
- Provide safe, comfortable and hygienic dialysis treatment.
- Monitor patients before, during and after dialysis treatment including measuring and recording stats, patient observations, and hemodialysis machine setup.
