About The Position

We are seeking a compassionate and dependable Patient Care Technician (PCT) to provide direct patient support in a long-term care and geriatric healthcare environment. The ideal candidate will assist patients with activities of daily living, support individualized care plans, and work collaboratively with interdisciplinary teams to ensure high-quality, patient-centered care.

Requirements

  • High School Diploma or equivalent required.
  • Current Patient Care Technician (PCT) Certification required.
  • Previous patient care, caregiver, nursing assistant, or healthcare support experience preferred.
  • Experience working with geriatric or long-term care populations preferred.
  • Experience with electronic medication administration records (eMAR) or QuickMAR systems preferred but not required.
  • Strong patient care and customer service skills.
  • Ability to assist patients with personal care needs while maintaining dignity and respect.
  • Excellent communication and interpersonal abilities.
  • Strong organizational and time-management skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Compassionate, patient-focused approach to care.
  • Ability to prioritize multiple responsibilities in a fast-paced healthcare environment.
  • Reliable attendance and dependable work ethic.
  • Basic computer and documentation skills.
  • Reliable transportation is required.
  • Candidates must be able to meet scheduling requirements without additional restrictions.

Nice To Haves

  • Experience with electronic medication administration records (eMAR) or QuickMAR systems preferred but not required.

Responsibilities

  • Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and mobility.
  • Support patient comfort, safety, dignity, and independence.
  • Observe, document, and report changes in patient condition to the appropriate healthcare team members.
  • Participate in patient orientation and assist with transitions into the care setting.
  • Coordinate and prioritize daily tasks in collaboration with nursing, housing, and clinical staff.
  • Assist with implementing individualized service and care plans.
  • Maintain accurate patient documentation and records.
  • Provide compassionate emotional support and positive interactions with patients and families.
  • Follow infection prevention, patient safety, and healthcare compliance standards.
  • Assist with routine patient monitoring and support services as assigned.
  • Support team operations and contribute to a collaborative work environment.
