Functions as part of the dialysis health care team in providing safe and effective home dialysis therapy and training for patients under the direct supervision of a licensed nurse. This role involves assisting in the setup of home dialysis equipment, maintaining a safe and clean working environment, and supporting the organization's commitment to quality improvement and patient satisfaction. The PCT will reinforce home hemodialysis (HHD) and/or peritoneal dialysis (PD) training, obtain patient vital signs and information, and maintain effective communication with patients, care partners, and healthcare team members. Responsibilities include managing treatment records, assisting with medication reconciliation, preparing laboratory specimens, and stocking supplies. The role also involves scheduling appointments, supporting patient registration for connected health, and maintaining logs. For Home Hemodialysis, the PCT will reinforce training, set up and troubleshoot HHD equipment, evaluate dialysis access, initiate and monitor treatments, and perform safety checks. For Peritoneal Dialysis, the PCT will reinforce training, troubleshoot PD cycler issues, perform phlebotomy, set up PD cyclers, and assist with exit site care. Staff-related duties include participating in recruitment, assisting with clinical orientation and training, functioning as a preceptor, conducting competency evaluations, and collaborating with management on quality standards and educational programs.
