The Home Therapy PCT II position requires CCHT certification and offers paid one-on-one training and education. The role involves empowering, engaging, and training dialysis patients on peritoneal and home hemodialysis using state-of-the-art programs and equipment. Primarily based in a clinic setting, the position also includes initial home visits conducted with another staff member present. This role functions as an onsite Patient Educator and Case Manager for patients with various shifts and work hours. The purpose is to function as part of the dialysis health care team in providing safe and effective home dialysis therapy and training for patients under the direct supervision of a licensed nurse, adhering to organizational policies, procedures, and regulations. The PCT II HT assists in setting up home dialysis equipment and maintaining a safe and clean environment within the home therapy program. They support the organization's commitment to the Quality Enhancement Program (QEP) and CQI Activities, actively participating in process improvement to help patients achieve QEP goals. All duties are performed under the direct supervision of the Home Therapies registered nurse (HT RN), in compliance with state laws, regulations, and policies.
