About The Position

We are recruiting for a pharmacist to join the Mount Carmel St. Ann's Pharmacy team to support our central pharmacy operations evening shift. This position will be a valuable member of the multidisciplinary teams operating in an innovative care setting to provide exceptional patient care. This pharmacist will serve as the medication expert in a variety of settings, interacting with physicians, nurses, medical residents, and other care team members daily. This position will also have the opportunity to contribute to the learning of Pharmacy Interns, APPE students. The Patient Care Pharmacist, within the scope of the Ohio Pharmacist Practice Act, will provide timely, accurate, cost effective, and patient-centric pharmaceutical care.

Requirements

Bachelor or Doctor of Pharmacy required
Current license to practice as a Registered Pharmacist in the State of Ohio.
Effective Communication Skills
Demonstrates the ability to plan, organize and manage pharmaceutical care, including delegating tasks to and providing direction to other members of the pharmacy team
Performs work independently with minimum supervision, and is effective in a stressful environment

Nice To Haves

PGY1 residency or three years of institutional pharmacy experience in an advanced clinical setting is preferred

Responsibilities

Coordinates the preparation, distribution, and dispensing of medication as appropriate in compliance with state and federal regulations and laws, accreditation standards, and departmental policies and procedures; directs and assists pharmacy support staff.
Maintains personal compliance and assists in maintenance of departmental compliance with continuous regulatory readiness.
Maintains records related to the transactions of the pharmacy, to achieve adequate control and accountability for all drugs, including controlled substances.
Assists in the management of pharmacy inventory by providing information to pharmacy purchasing, removing expired contaminated or deteriorated stock when detected, helping maintain perpetual inventories, and processing non-formulary requests.
Interprets, evaluates and verifies all medication orders.
Collaborates with multidisciplinary team in order to assure optimal and cost effective drug therapy. This includes adjusting drug dosing and schedules, recommending the most efficacious therapy, practicing stewardship in antibiotics, performing drug consults and monitoring, and detecting possible adverse drug reactions, food/drug interactions, drug/drug interactions, and allergic cross reactivity.
