About The Position

We are hiring a Patient Care Navigator RN for our Saint Luke's Neuroscience program! This is a hybrid opportunity with working hours of Monday - Friday, 8am - 4:30pm. The role involves navigating patients through their diagnosis and treatment, ensuring timely services. This includes meeting with patients and families to provide emotional support, education, and guidance from post-biopsy through end-stage disease. The navigator will provide medical information, educate on pathology reports, and connect patients with community resources. Responsibilities include facilitating rapid scheduling and follow-up tracking to ensure treatment plan completion, identifying and resolving service gaps, and promoting the Navigator role and cancer awareness through community presentations and participation in cancer-related organizations. The role also coordinates and provides education for newly diagnosed cancer patients and supports clinic nurse roles.

Requirements

  • Strong background in Neurology, required
  • MO and KS RN license required
  • 2 years Applicable Experience
  • Registered Nurse - Various
  • Bachelor's Degree

Responsibilities

  • Navigates patients through their diagnosis and treatment ensuring timely services.
  • Meet with patient and families to provide emotional support, education and guidance to all patients post-biopsy through end stage disease if necessary.
  • Provide medical information, education and review of pathology reports and knowledge of community resources.
  • Responsible for facilitating rapid scheduling and follow-up tracking of patients to ensure completion of their recommended treatment plan.
  • Identify and problem solve any service gaps identified.
  • Promote the Navigator role and cancer prevention, detection and treatment awareness to the community through presentations and participation in cancer related organizations.
  • Coordinates and provides education for all newly diagnosed cancer patients from our campus and outside community.
  • Provides support to clinic nurse roles and maintains many additional job functions as needed.
