About The Position
Working in partnership with other case managers, physicians and patients, you will identify and assist patients with complex post-acute HIV needs and facilitate the development of a safe discharge plans. You will be responsible for assessing the patient’s continuum of care needs, developing a plan in collaboration with the patient the healthcare team, and implementing the plan using the resources of the Resource Center. You will advocate for the patient and family throughout the entire process.
Requirements
- Bachelor’s degree in Nursing with RN licensure in the State of Michigan OR Bachelor’s degree in Social Work with licensure as a Social Worker in the State of Michigan.
- A minimum of three years experience in HIV care.
- Other clinical or behavioral disciplines with comparable experience will be considered.
- Employees must be vaccinated or have lab confirmed immunity for Measles, Mumps, Rubella and Varicella.
- Employees must receive a flu vaccine during the flu season in the year that they are hired and annually thereafter, or receive an approved medical or religious exemption.

- A Master’s degree in Social Work is preferred.
