About The Position
The Home Health Patient Care Manager is responsible for supervising and coordinating clinical services and an interdisciplinary staff team to ensure continuity of high-quality care for assigned home health patients. Care is delivered in accordance with physician-prescribed plans of care and applicable state and federal laws and regulations. The manager provides nursing services within the scope of practice, coordinates referrals and clinician assignments, assesses patient needs and eligibility, and reviews assessments and plans of care. The role requires the ability to function in home situations involving clients of varied backgrounds and circumstances.
Requirements
- Current unrestricted RN licensure in the state of practice.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
Nice To Haves
- Current CPR certification or ability to complete it within 90 days of hire.
- Home care experience.
Responsibilities
- Provide clinical services within the scope of nursing practice and according to the plan of care, in coordination with other members of the health care team.
- Receive referrals, ensure appropriate clinician assignments, evaluate patient orders, and schedule start-of-care visits.
- Coordinate determinations of patient home health benefits, medical necessity, and ongoing insurance approvals.
- Ensure patient needs are continually assessed and that care is individualized, appropriate, reasonable, meets home health eligibility criteria, and follows physician orders.
- Review assessments and plans of care daily according to assigned workflow, and consult clinicians with recommendations as appropriate.
- Function in home situations regardless of the client's age, race, creed, color, sex, disability, or financial condition.
