About The Position
DaVita's Patient Pathways team is seeking a "Shining Star" for a contract role at Cedars-Sinai Medical Center. This position is perfect for an RN who thrives on autonomy and clinical coordination. You will serve as the primary link between patients, families, and elite physician teams, ensuring that patients with chronic kidney disease (CKD/ESRD) receive seamless, high-quality care. The role offers project stability, prestige, a structured lifestyle with a guaranteed M-F, 8-5 schedule, and the opportunity to function as a clinical "Change Agent," managing outcomes and reducing hospital readmissions through expert care coordination.
Requirements
- Current, good-standing CA Registered Nurse (RN) license.
- Minimum of 3 years in clinical nursing.
- Background in healthcare performance coaching or acting as a "change agent."
- Valid CA driver’s license with a positive record.
Nice To Haves
- Renal nursing experience
- BSN
- Certifications like CNN or CCM
Responsibilities
- Work directly within the hospital to facilitate quality, cost-effective care transitions for renal patients.
- Act as a dedicated advocate to ensure continuity of care between the hospital, the family, and the outpatient provider.
- Apply CQI techniques and clinical data to drive better patient satisfaction and health outcomes.
