About The Position
VNA Hospice NWI is seeking a dedicated Patient Care Manager (PCM) RN to guide their interdisciplinary team and ensure exceptional care for every patient they serve. This role involves leading and mentoring a team of nurses, aides, social workers, and chaplains, overseeing patient care plans, ensuring regulatory compliance, and supporting families with empathy and professionalism. The PCM RN will also collaborate closely with physicians and clinical leadership, fostering a culture of compassion, accountability, and excellence.
Requirements
- RN license in the state of Indiana
- Strong communication and organizational skills
- A heart for service and a commitment to quality care
Nice To Haves
- Hospice or home health leadership experience preferred
