About The Position

The Patient Care Facilitator plays a crucial role in ensuring the smooth flow of patients and continuity of care within the hospital. This position involves performing direct patient care tasks, managing clerical duties, utilizing communication techniques effectively, and adhering to hospital policies and procedures. The role also includes specific responsibilities related to patient navigators, EMR usage, and email/telephone etiquette. The position requires an Associate degree and offers a demanding, fast-paced office environment with direct patient contact, involving moderate to heavy physical activity.

Requirements

  • Associate degree at minimum
  • Previous experience in a medical setting or 2 years relevant work experience preferred
  • Excellent communication/ interpersonal skills
  • Bilingual preferred
  • Knowledge of computer technology preferred
  • Successful completion of St. Barnabas Hospital orientation

Responsibilities

  • Perform tasks as specified by Director of Care Transitions leadership team.
  • Maintain an orderly flow of patients.
  • Follow OSHA standards and Infection Control policies.
  • Seek direction and supervision when necessary, but function independently where appropriate.
  • Correctly identify all patients by dual identifier prior to performing any procedures.
  • Demonstrate flexibility with changing assignments due to staffing needs and adequately manage workload.
  • Perform related clerical duties to ensure continuity of patient care.
  • Utilize computer technology in the work area.
  • Enter data accurately.
  • Discharge patients and make referrals as indicated by the team.
  • Maintain a safe environment for patients and report unsafe conditions to the unit manager.
  • Utilize communication techniques effectively.
  • Demonstrate a professional, caring, respectful approach with patients, families, and co-workers.
  • Understand and respect cultural differences.
  • Communicate in a pleasant, courteous, respectful manner always.
  • Maintain confidentiality and privileged communication.
  • Use effective communication to assist in directing patient flow.
  • Assume responsibilities in knowing St. Barnabas’ Policy and Procedures and Standard of Care.
  • Participate in staff meetings.
  • Report promptly to duty.
  • Comply with St. Barnabas attendance policies.
  • Follow dress code policies and wear ID badge.
  • Participate in the Department Quality Improvement Program.
  • Support the Mission and philosophy of St. Barnabas Hospital.
  • Participate in the development of staff members.
  • Meet annual health assessments requirements.
  • Identify learning strengths and needs.
  • Utilize staff meetings.
  • Other duties assigned.
  • Modify approaches based on age-specific needs and responses to treatment for adult and geriatric populations.
  • Use communication techniques that are age-appropriate for adult and geriatric populations.
  • Complete readmission check sheets in a timely manner.
  • Provide patients with adequate resources such as housing, food, and legal services.
  • Refer to community agencies.
  • Re-assess patients that are re-admitted to a medical unit within 30 days of last discharge for COPD and/or CHF.
  • Assess patients in the ED with COPD and/or CHF to avoid admission if possible.
  • Document patient care navigator notes in EMR (EPIC).
  • Respond to incoming emails and calls in a timely manner.
  • Ensure out-of-office notifications are activated on scheduled days off.
  • Review emails and voicemails on a daily basis.
