About The Position

As a Patient Care Coordinator, you will work as part of our local sales team and report to a Market Executive of Sales. Your role will involve coordinating care from Hospitals and SNFs in the territory. You will meet with patients at the bedside to discuss home health services, disciplines, and what to expect at home. Additionally, you will support operations teams by ensuring they have the necessary resources, information, and coordination for a successful start of care. You will also assist with sales calls as needed to support the Account Executive in addressing client needs and advancing business objectives.

Requirements

• LPN, LVN or RN license is required.
• Knowledge of governmental regulations, Medicare eligibility requirements, comprehensive understanding of potential care plan needs for the patient and coordination of necessary resources.
• Travel within assigned geographical territory is required.
• This role is considered patient facing and is part of Human's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

Nice To Haves

• Three years clinical experience is preferred.
