About The Position

The Patient Care Coordinator (PCC) collaborates and ensures professional nursing practice in a clinical setting by applying the nursing process, assessing the patient's clinical condition while prescribing appropriate nursing actions to meet the needs of the patient, assuring quality patient care utilizing standards of practice established by the facility, providing educational opportunities with patients and other significant individuals, and communicating effectively using both verbal and written abilities. The PCC works closely with the clinical manager in planning, coordinating and evaluating the activities of the patient care unit. Participates as a leader in the customer service program, champion for rounding, service recovery, and is aware of employee and customer satisfaction scores. The PCC oversees completion of unit duties during shift, attends unit and shift supervisor meetings and may provide direct patient care dependent upon staffing needs of the unit.

Requirements

  • Registered Nursing license
  • Demonstrates the knowledge and skills necessary to perform the job effectively in accordance with policy and procedures.
  • Registered Nurse (RN) license to practice in the state or as listed in the Nurse Licensure Compact (NLC)
  • Associate's Degree or Bachelor's Degree- For external candidates, required upon hire
  • Bachelor's Degree- For internal candidates, obtain BSN within 3 years of promotion to position.
  • 3 years Nursing/Patient Care experience
  • Basic Life Support Program (BLS) - American Heart Association
  • Unit Specific Certifications - Franciscan Alliance (if required by unit)
  • Advanced Cardiovascular Life Support (ACLS) - American Heart Association (if required by unit)

Nice To Haves

  • Advanced ICU skills such as IABP, Prisma (CRRT) Impella
  • More than one year of ICU experience
  • 1 year Area of practice or related specialty

Responsibilities

  • Applies the nursing process consistently in the delivery of patient care.
  • Collaborates with nursing staff, interdisciplinary staff, and teams, leaders, and others.
  • Attains and maintains knowledge and competency that reflects current nursing practice.
  • Collects comprehensive data pertinent to the patient's health or situation in a systematic and ongoing process, involving the patient, family, or other Healthcare providers as appropriate.
  • Completes assignments in a thorough, accurate, and timely manner that achieves expected outcomes.
  • Demonstrates a commitment to lifelong learning and ongoing professional development through such activities as continuing education, certification, and participation in professional organizations.
  • Develops an individualized care plan that prescribes strategies and alternatives to attain expected patient outcomes.
  • Evaluates and monitors the quality of nursing care administered by the nursing staff on the unit.
  • Identifies oncoming shift's staffing needs and initiates action that assists in procuring adequate levels of staffing within their control.
  • Performs regular rounding for outcomes on patients and staff.
  • Prepares and delivers the end of shift reports to oncoming shift Charge Nurse.
  • Assesses the patient's clinical condition accurately while prescribing appropriate nursing actions to meet the needs of the patient.
  • Displays ingenuity, self-reliance, resourcefulness, assertiveness, ambition, and anticipation of what needs to be done and follows through with assignments.
  • Takes needed action without direct instructions.
  • Participates in committees, employee events, and activities.
  • Prioritizes data collection activities based on the patient's immediate condition or anticipated needs.
  • Remains current and shows compliance regarding new developments and technology in areas of responsibility, including documentation, medication administration, monitoring systems, etc.
  • Utilizes knowledge of federal, state and accreditation standards and regulations in the unit setting.
  • Performs timely and effective service recovery to address patient/family complaints and grievances.
  • Participates in people management activities for direct team members such as conducting performance evaluations, disciplinary actions, and interviews.
