About The Position

Soleo Health is seeking an in-office Patient Care Coordinator/Scheduler to support our Specialty Infusion Suite in Dublin, OH. This role involves data entry of clinical information, coordinating scheduling activities for the nursing department, responding to calls, promoting agency programs, providing information, and processing referrals. The position requires skills in planning, organizing, and coordinating patient care delivery, ensuring schedules are accurate and efficient. The Patient Ambassador also engages directly with patients post-visit to coordinate next steps in care, ensuring clarity of future appointments and supporting a positive patient experience. Requirements

High school diploma or equivalent required
Prior healthcare scheduling experience required
Home infusion experience required
Ability to work on-site and interact directly with patients in a clinical setting.
Strong communication skills
Ability to prioritize and multitask
Basic computer skills including Microsoft Excel, Word, Outlook

Nice To Haves

Licensed Practical Nurse (LPN) preferred
Healthcare scheduling experience preferred
CPR Plus application experience preferred

Responsibilities

Receive medication referrals and collect insurance information through various methods.
Proficient data entry and generating daily office communications.
Contact referral sources, patients, or prescriber’s offices to obtain additional information required for verification of benefits or prior approvals.
Place outbound calls to patients or doctor’s offices to notify them of care status.
Collaborate with prescribers to facilitate payor denial appeals.
Act as a single point of contact for all referral activity with internal customers, manufacturers/HUBs, patients, prescribers, and referral sources.
Provide exceptional customer service to external and internal customers, resolving requests in a timely and accurate manner.
Ensure appropriate notification of patients regarding financial responsibility, benefit coverage, and payor authorization for services.
Serve as the main point of contact for the nursing department, demonstrating the ability to multi-task and remain calm under pressure.
Coordinate and plan scheduling activities for the nursing department, ensuring accuracy and efficiency.
Demonstrate knowledge of the geographical area for efficient scheduling and coordination of nursing visits.
Clearly communicate patient needs for external agency support, when necessary, to ensure timely care.
Collaborate with internal departments and external partners to promote, foster, and maintain optimal relationships across departmental and company lines.
Support client satisfaction at a level that ensures account retention.
Meet with patients in person following AIS visits to schedule their next appointment prior to departure.
Confirm patient understanding of their next visit and ensure accurate scheduling to support timely medication dispensing and care coordination.
Inquire about the patient’s appointment experience and request completion of a brief satisfaction survey.
Serve as a key touchpoint for enhancing the in-clinic patient experience and supporting overall customer satisfaction outcomes.
