About The Position

Become a part of our caring community and help us put health first As a Patient Care Coordinator, you will: Follow up on Face-to-face requirements, educating physicians on CMS guidelines for Home Health in person, or virtually by email or phone. Visit different VA offices in NKY/Cincinnati area to obtain orders/signatures and follow up on VA patients. Attend Tuesday business development meetings, bringing relevant data, reports, and information about changes within accounts and referral sources. Report to a Market Executive of Sales Work an 8 hour shift every Tuesday/Thursday or Tuesday/Friday Use your skills to make an impact

Requirements

• RN/LPN/LVN license.
• Three years of nursing experience is preferred
• Must have knowledge of governmental regulations, Medicare eligibility requirements, comprehensive understanding of potential care plan needs for the patient and coordination of necessary resources.
• Excellent customer service, account development capabilities, organization, time management, problem-solving, communication and selling skills.
• We require travel within an assigned territory.
