About The Position

As the Patient Care Coordinator, you will be responsible for developing and maintaining both the staffing and scheduling systems in coordinating patient care in a timely and accurate manner. The Patient Care Coordinator is responsible for processing workflow related to the scheduling of patient services of all disciplines across the Home Care and Hospice Agency while following quality and patient safety measures.

Requirements

• 1 year of clerical and staffing experience in a healthcare setting preferred.
• High School or GED.
• 2 Year/Associate's degree preferred.

Responsibilities

• Developing and maintaining staffing and scheduling systems for patient care coordination.
• Processing workflow related to the scheduling of patient services for all disciplines.
• Ensuring quality and patient safety measures are followed in patient care coordination.
