About The Position
UW Medical Center- Heart Institute Clinic has an outstanding opportunity for a Patient Care Coordinator Supervisor. This is a full-time, 100% FTE (40 hours per week) Day Shift position, Monday-Friday, and is an on-site role with no telework. Requirements
- A Bachelor’s Degree in a medically related or public relations field
- Two years of experience in a tertiary patient care setting involving interaction with a variety of medical services, contact with patients and families, interaction with physicians, and proficiency in medical terminology.
- Equivalent education and/or experience may substitute for minimum qualifications except when there are legal requirements, such as a license, certification, and/or registration.
Responsibilities
- Supervise assigned staff ensuring compliance with regulatory agencies, insurance and other financial requirements, and hospital policies and procedures
- Act as a liaison between patients, consulting physician, referring physician, or outside agency to coordinate patient appointments, admission, or surgery
- Coordinate the exchange of information either written or verbally before and after patients are seen
- Consistently demonstrates productive and timely communication with charge nursing staff, peers, patients, physicians, volunteers and other medical center departments and ancillary service
- Increase collaboration between our PCC/PSS team through regular meetings and addressing overlap and delegation of duties between the two teams
- Oversee break and vacation coverage through equitable means
- Report out weekly, AccuReg, phone calls, referral numbers to administration
- Coordinate pre-admission test and procedures
- Schedule and ensure pre-anesthesia work ups are completed
- Coordinate the direct transfer of patients from outside hospitals to UWMC with physicians, medial staff and Admitting
