About The Position

The Patient Care Coordinator (PCC) is a vital member of the hospice care team, responsible for ensuring the delivery of compassionate, patient-centered care to hospice patients and their families. This role reports to the Director of Clinical Services and involves overseeing and coordinating clinical services to ensure quality, continuity, and consistency of care in alignment with hospice philosophy and regulatory requirements.

Requirements

  • Current Registered Nurse (RN) license in the state of Texas.
  • Minimum of 2 years nursing experience, preferably in hospice or palliative care.
  • Strong organizational and time management skills, with the ability to handle multiple tasks efficiently.
  • Excellent communication and interpersonal skills, with a compassionate and patient-focused approach.
  • Proficiency in electronic medical records (EMR) and other healthcare-related technologies.
  • Proficient in running, analyzing, and interpreting reports to identify trends and insights.
  • Able to leverage data to inform and implement practice changes, enhancing patient care outcomes.
  • Strong ability to use data-driven decision-making to support continuous improvement in hospice care services.

Nice To Haves

  • Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) with relevant experience will be considered.
  • Certification in Hospice and Palliative Nursing (CHPN) preferred.
  • Experience in care coordination case management, or supervisory roles in a similar role is preferred.

Responsibilities

  • Coordinate and monitor the day-to-day delivery of patient care services, ensuring that all aspects of the patient's care plan are carried out effectively and efficiently.
  • Collaborate with interdisciplinary team members, including physicians, social workers, chaplains, and therapists, to develop and update individualized care plans.
  • Act as a liaison between patients, families, and the hospice care team to ensure clear communication and address any concerns or needs promptly.
  • Provide clinical supervision and support to nursing staff, including licensed practical nurses (LPNs) and certified nursing assistants (CNAs), ensuring adherence to hospice policies, procedures, and best practices.
  • Participate in case conferences, clinical meetings, and quality improvement initiatives.
  • Assist in the management of patient admissions, discharges, and transfers, ensuring accurate documentation and regulatory compliance.
  • Offer emotional support and guidance to patients and their families, helping them navigate the end-of-life journey with dignity and respect.
  • Educate patients and families about the hospice care process, medications, and symptom management.
  • Assist the Director of Clinical Services in monitoring and maintaining high standards of care, identifying areas for improvement, and implementing corrective actions as needed.
  • Ensure compliance with all state, federal, and accrediting agency regulations, including proper documentation and reporting.
  • Provide ongoing education and training to hospice staff on clinical care practices, hospice philosophy, and regulatory requirements.
  • Participate in professional development opportunities to stay current with best practices in hospice care.
