About The Position
NHC Scott is now hiring a full-time Patient Care Coordinator RN. This role is responsible for managing the fall program, coordinating patient transportation and appointments with insurance, participating in a rotating on-call schedule (1 in 5 weekends), and supporting the day-to-day operations of the nursing team. The position also involves overseeing hospital transfers and readmissions, and assisting the Infection Control nurse. The work hours are Monday to Friday, 7:00 am - 3:30 pm, with flexible/negotiable hours. The on-call duties include weekends from Friday 5:00 pm to Monday 8:00 am.
Requirements
- Graduate from a nursing program accredited by the State.
- Maintain a current unencumbered license to practice in the state where employed.
Responsibilities
- Manage the fall program.
- Work with insurance regarding patient transportation and appointments.
- Participate in a rotating on-call schedule (1 in 5 weekends, Friday 5pm-Monday 8am).
- Support the day-to-day nursing team.
- Oversee hospital transfers and readmits.
