About The Position

Become a part of our caring community As a Patient Care Coordinator, you will: Work primarily in the field as part of our sales team, reporting to a Market Executive of Sales. Develop/maintain contact with key hospital, skilled nursing, assisted living facility discharge planning services and/or management to provide ongoing updates on CenterWell's services Meet with patients and discharge planners at hospital/facility to arrange discharge. Cover an area that includes Springfield and Branson areas. Use your skills to make an impact

Requirements

• RN/LPN/LVN license required.
• Must have knowledge of governmental regulations, Medicare eligibility requirements, comprehensive understanding of potential care plan needs for the patient and coordination of necessary resources.
• Must have excellent customer service, organization, relationship building and communication skills.

Nice To Haves

• Bachelor's of Science in Nursing preferred
• Three years of clinical experience preferred.
