About the position
The Patient Care Coordinator (Better Concierge) is responsible for assisting patients and providers throughout their medical supply journeys, ensuring a seamless experience from product selection to insurance handling and supply delivery. This role requires a problem-solving mindset and a willingness to think outside the box in order to improve the quality of life for patients. The objective is to provide a five-star hotel level of service in the world of medical supplies. Reporting to the Strategy & Operations Lead, the Patient Care Coordinator will be the main point of contact for patients, providers, and payers, managing complex processes and collaborating with the care team to connect patients with necessary support and resources.
Responsibilities
- Be the first and last one to talk to patients about their needs, providers about their referrals, and payers about plan coverage
- Work with urgency daily between patients, providers, and payers to get patients needed medical supplies
- Manage complex processes across teams, connecting the dots to do whatever is needed to get the job done
- Use ticketing tools and manual processes to make sure nothing falls through the cracks
- Liaise with our care team to connect patients with peer support and educational resources to support their condition management
- Strategize with our team to collaborate on ways to improve internal functions and improve outcomes
