Requirements

  • High School Diploma, GED, or STNA
  • De-escalation training within 6 months and annually
  • BLS within 90 days of hire

Responsibilities

  • Provides high quality, direct patient care to an assigned group of patients.
  • Assists with activities of daily living.
  • Utilizes safe patient handling equipment.
  • Measures and documents intake and output.
  • Performs foley removal and external urinary catheter management.
  • Sets up telemetry monitors.
  • Performs blood glucose monitoring.
  • Sets up oxygen and performs tasks delegated per scope of practice and skill set.
  • May provide clerical support to the care team, acting as a liaison to facilitate communication with or between other care providers and visitors directly or via telephone.
  • Works collaboratively with other care team members, including the patient and family.
  • Participates collaboratively with RN to achieve optimal outcomes.
  • Communicates with all team members regarding care provided to the assigned patient group.
  • Carries out daily activities under the direct supervision of a Patient Care Coordinator / Registered Nurse, RN or Assistant Nurse Manager and indirect supervision of the Nurse Manager or Director.
