Requirements
- High School Diploma, GED, or STNA
- De-escalation training within 6 months and annually
- BLS within 90 days of hire
Responsibilities
- Provides high quality, direct patient care to an assigned group of patients.
- Assists with activities of daily living.
- Utilizes safe patient handling equipment.
- Measures and documents intake and output.
- Performs foley removal and external urinary catheter management.
- Sets up telemetry monitors.
- Performs blood glucose monitoring.
- Sets up oxygen and performs tasks delegated per scope of practice and skill set.
- May provide clerical support to the care team, acting as a liaison to facilitate communication with or between other care providers and visitors directly or via telephone.
- Works collaboratively with other care team members, including the patient and family.
- Participates collaboratively with RN to achieve optimal outcomes.
- Communicates with all team members regarding care provided to the assigned patient group.
- Carries out daily activities under the direct supervision of a Patient Care Coordinator / Registered Nurse, RN or Assistant Nurse Manager and indirect supervision of the Nurse Manager or Director.
