About The Position

The Patient Care Assistant supports the department by providing direct patient care, including clinical and administrative duties. This role involves being cross-trained to perform duties outlined in Track 1 and Track 2. Track 1 focuses on completing medical/administrative tasks supporting the medical provider, while Track 2 involves working with the Medical Care & Dental Care Department Interdisciplinary Team to provide clinical and administrative assistance to patients.

  • High school diploma/GED required

Responsibilities

  • Maintain effective communication with patients regarding wait times, needs, expectations, and feedback.
  • Escort patients to various areas within the medical office, including check-in/out, lab, and exam rooms.
  • Complete all tasks sent via EHR within the requested timeframe.
  • Ensure compliance with health center policies and procedures related to medical examinations.
  • Ensure proper and timely check-in of patients and prepare them for medical examination.
  • Assist with the coordination of patient flow.
  • Ensure proper patient identification using a minimum of two patient identifiers.
  • Schedule follow-up appointments for patients.
  • Complete documentation in patient charts accurately and in a timely manner.
  • Receive/send documents via fax as requested by the provider.
  • Complete Medical Record Release forms.
  • Make phone calls and document in EHR per provider request.
  • Record pertinent information on forms.
  • Ensure proper and smooth operational flow related to patient visits, prioritizing based on medical needs.
  • Review and upload patient vital signs in the EHR.
  • Update and record patient information in the EHR, including allergies, medication reconciliation, immunizations, and history forms.
  • Prepare and review the next day's patient schedules and care requirements to maintain patient flow.
  • Assist with the completion of forms such as Women Infant Children (WIC), pre-operative Surgical, work Excuse/return to work, and proof of pregnancy.
  • Ensure proper Patient Health Questionnaire-9 (PHQ-9) and depression screening for suicide risk for every new patient.
  • Collect and accurately document patient vital signs in the EHR (height, weight, temperature, heart rate, blood pressure, pulse oximeter, pain level).
  • Report abnormal findings to the Registered Nurse and/or Medical Provider.
  • Prepare venipuncture forms.
  • Prepare Advance Beneficiary Notice (ABN) forms and inform patients of associated costs.
  • Prepare and complete lab orders/requisitions.
  • Ensure proper collection, labeling, and processing of all specimens using a minimum of two patient identifiers before drawing blood.
  • Assist Medical Providers with medical procedures/testing, including EKGs, vision and hearing tests, diabetic foot/monofilament tests, fall risk assessments, waived testing, and documentation.
  • Review visit summaries with patients upon discharge by the provider.
  • Ensure proper processing and secure enclosure of specimens for transport to the lab.
  • Prepare rooms for clinical examination and patient procedures.
  • Ensure materials, supplies, and equipment are stocked and available.
  • Assist Medical Providers with procedures/testing and ensure proper documentation in the medical record (e.g., Pap smears, colposcopy, biopsy, pelvic/breast exams, nebulizer treatments).
  • Obtain and review medical records of all diagnostic/lab reports.
  • Collect, transport, disinfect, and prepare instruments for sterilization.
  • Perform high-level disinfection (HLD) and sterilization of instruments.
  • Ensure complaints, history of current illnesses, problem lists, health reminders, patient history, social history, family history, medical/surgical history, and gauge for recent hospitalizations are entered into the patient's EHR.
  • Perform PHQ-9 Depression screenings.
  • Place orders for laboratory tests, standing orders, referrals, new medications/refills, vaccines, and injections.
  • Ensure proper ICD-10 coding/charges by diagnosis as per medical provider's instructions.
  • Enter Uniform Data System (UDS) & Meaningful Use (MU) measures.
  • Enter self-management goals and plan of care.
  • Provide accurate and timely documentation within the same day of intervention.
  • Ensure examination rooms are in compliance with OSHA and Joint Commission standards.
  • Ensure proper documentation/logging and disposal of medication into hazardous/black boxes.
  • Receive training and participate in competency assessments.
  • Ensure cleanliness of clinic area and examination rooms.
  • Report cleaning or safety observations to the registered nurse and/or office manager.
  • Maintain agency guidelines related to safety, outreach, and confidentiality.
  • Ensure proper hand washing according to CDC guidelines.
  • Maintain and follow infection control standards and Universal Precautions.
  • Provide feedback on risk management/safety assessments to the Registered Nurse and/or office manager.
  • Understand and appropriately act upon assigned role in Emergency Code System.
  • Understand and perform assigned role in Agency Continuity of Operations Plan (COOP).
  • Greet internal or external customers with courtesy, making eye contact, and responding with appropriate tone and nonverbal language.
  • Listen attentively to customers, reassure understanding of requests, and provide appropriate options or resolutions.
  • Provide services by following established protocols and seeking assistance when needed.
  • Take initiative to anticipate customer needs, engage them in the process, and follow up as needed.
  • Prioritize customer requests to ensure prompt and effective responses.
  • Participate in training sessions and other meetings as required.
  • Participate in agency developmental activities as requested.
  • Perform other duties as assigned.
