About The Position

Working under the guidelines of Hamilton Health Center’s patient-centered model of care, the Patient Benefit Services Coordinator serves as a liaison for uninsured and underinsured patients, providing eligibility screening, enrollment assistance, education, and outreach related to Medicaid, CHIP, and Marketplace insurance programs. This role supports access to care by assisting patients with insurance applications, financial assistance options, and referrals while participating in in-reach and outreach activities to improve engagement in evidence-based care.

Requirements

  • High school diploma or GED required.
  • Minimum of two (2) years of experience working with children and families in public health, outreach, social work, or a related field.
  • Reliable transportation required to travel throughout the service area.

Nice To Haves

  • Coursework or degree from an accredited college or university in Social Work, Public Health, Public Administration, or a related field preferred.
  • Working knowledge of Pennsylvania Medicaid, CHIP, and the COMPASS benefit application system preferred.

Responsibilities

  • Provide fair, impartial, accurate, and culturally appropriate information regarding health insurance coverage options.
  • Screen patients for eligibility and assist with applications, renewals, and enrollments for programs administered by the Center, including Medicaid, CHIP, Presumptive Eligibility, Medicare Part D, PACE/PACENET, low-income subsidy programs, and prescription assistance programs.
  • Assist patients with completing electronic enrollment applications through the Pennsylvania COMPASS system.
  • Educate patients and families on the benefits of enrolling in low- or no-cost health insurance programs available through the Commonwealth of Pennsylvania.
  • Maintain current knowledge of Medicaid, CHIP, Marketplace plans (QHPs), and related eligibility and enrollment requirements.
  • Conduct daily in-reach and outreach activities to existing patients and community residents to promote awareness of coverage options.
  • Represent Hamilton Health Center at community events as assigned by leadership.
  • Assist with the development of outreach, marketing, and educational materials related to enrollment and benefit services.
  • Provide linguistically appropriate services, including support for individuals with limited English proficiency, and ensure accessibility for individuals with disabilities.
  • Refer patients with complaints, grievances, or complex coverage issues to appropriate consumer assistance programs, ombudsman offices, or state agencies.
  • Maintain strict confidentiality and comply with HIPAA requirements to protect personal, confidential, and identifiable information.
  • Collect and analyze service data and prepare required monthly reports.
  • Participate in required trainings, conference calls, and workshops sponsored by PACHC and other partners.
  • Perform other job-related duties as assigned.
