The Patient Assessment Coordinator is responsible for coordinating the process of providing accurate and timely data for the Prospective Payment System and quality reporting program for Medicare patients. This role oversees all aspects of data collection on the IRF/PAI, ensures supporting documentation is present, and provides staff education on CMS guidelines. The Coordinator reviews each IRF/PAI form for completeness, accuracy, and timeliness, monitors the accuracy of functional status scoring and reflection of patient care needs, and inputs appropriate information into the eRehabData system. This role also coordinates the collection of patient information from the medical record and completion of assigned IRF/PAI sections. Additionally, the Coordinator sets IRF/PAI completion dates for the multidisciplinary team, develops and interprets policies and procedures, and serves as the primary resource for problem-solving related to Medicare’s IRF prospective payment system, coverage and classification criteria, and quality reporting program requirements.
