About The Position

Kindred Hospital San Antonio Central, located inside Baptist Medical Center, is a long-term acute care (LTAC) hospital specializing in the treatment of patients recovering from post-intensive care and medically complex conditions. Our hospital provides both intensive care and telemetry-level services, offering advanced monitoring and support for patients requiring extended medical care. With a multidisciplinary team dedicated to personalized treatment plans, we focus on helping patients achieve significant recovery and transition to the next level of care. This role involves conducting patient assessments for potential admissions, maintaining positive relationships with referral sources and internal teams, and coordinating the transmission of clinical and benefits information for admissions into ScionHealth Specialty Hospitals. The coordinator will collect, organize, evaluate, and communicate pertinent clinical information, demonstrating strong customer service, attention to detail, and a sense of urgency. Confidentiality and regulatory compliance are paramount.

Requirements

  • Ability to collect, organize, evaluate and communicate pertinent clinical information with effective verbal and written skills.
  • Strong customer service skills and commitment to service excellence.
  • Computer skills that include Microsoft Office, Word, Excel and Outlook and PowerPoint.
  • Excellent presentation and communication skills.
  • Clinical Background with ability to conduct Patient Assessments.
  • Ability to work under stress and to respond quickly in crisis situations.
  • Knowledge of healthcare marketing and plan development.
  • Ability to work cooperatively as a member of a team.
  • Must read, write and speak fluent English.
  • Must have good and regular attendance.
  • Associate or Bachelor’s degree.
  • Demonstrated understanding of Medicare, Medicaid, and other payers as related to LTACH benefits.

Nice To Haves

  • Degree in healthcare related field preferred.
  • Healthcare professional licensure preferred.
  • Registered Nurse, Licensed Practical Nurse, Respiratory Therapist, Physical Therapist, Occupational Therapist, or Social Worker license.
  • Two (2) years clinical patient care/treatment experience in healthcare field preferred.

Responsibilities

  • Conduct patient assessments in accordance with company standards for appropriate patients for potential admission into the system.
  • Maintain positive relationships with referral sources, sales teams, admission, managed care, clinical and operational teams.
  • Coordinate transmission of clinical and benefits information of patients for possible admission into our ScionHealth Specialty Hospitals.
  • Collect, organize, evaluate, and communicate pertinent clinical information with effective verbal and written skills.
  • Operate with a sense of urgency to produce quick results.
  • Maintain all patient information in a manner that ensures confidentiality and meets all regulatory requirements.
  • Work closely with patients and their family members to educate them on the benefits of Kindred Hospital.
  • Assist in conducting tours of the hospital with families and referral sources as needed.
  • Facilitate the timely admission of patients from the referral sources: Identifies medical record information needed from referring facility; Completes the Pre-Admission Clinical Evaluation (PACE) form to assist in evaluating appropriateness of admission; Facilitates and coordinates physician-to-physician communication as necessary; Identifies specific patient needs for equipment, supplies and consult services.
  • Meets with management staff, participates in marketing meetings and serves on various internal committees as assigned.
  • Maintains a state-of-the-art knowledge of areas of responsibility and develops and follows a program of continuing education.
  • Conducts job responsibilities in accordance with the standards set out in the Company’s Code of Business Conduct, its policies and procedures, the Corporate Compliance Agreement, applicable federal and state laws, and applicable professional standards.
