About The Position

The Patient Advocate assists the Director of Performance and Risk Management, leadership, medical staff and PI Committee in carrying out and implementing a comprehensive and integrated program. Fulfill all of the functions of the patient advocate. This role is responsible for contacting any patient requesting a patient advocate within the first working day of the request, and any individual filing a patient grievance within the first working day following the receipt of the grievance. The Patient Advocate follows the patient grievance process, ensuring the investigation of the grievance, appropriate actions are taken, documenting actions, and informing the patient/representatives of the results of the investigation and actions taken. This role also maintains the grievance log, ensures that patient rights and responsibilities are posted, answers patient questions about rights, and ensures that patient’s rights are respected. The Patient Advocate is responsible for completing required reports and submitting them as assigned, notifying of all compliance issues, and preparing all reports as assigned by the Director.

Requirements

  • Bachelor’s degree or equivalent in Nursing, master's in social work or counseling preferred.
  • 1-2 years’ experience in a healthcare or behavioral health setting involving quality improvement preferred.
  • Computer skills involving knowledge of Excel spreadsheets and business writing.

Responsibilities

  • Contact any patient requesting patient advocate with-in the first working day of request.
  • Contacts any individual filing a patient grievance with-in the first working day following the receipt of the grievance.
  • Follows the patient grievance process ensuring the investigation of the grievance, ensuring appropriate actions are taken, documenting actions and informing the patient/representatives of results of investigation and actions taken.
  • Maintain the grievance log.
  • Ensure that the patient rights and responsibilities are posted, answers the patients questions about rights, and ensures that the patient’s rights are respected.
  • Responsible for completing required reports and submitting as assigned.
  • Responsible to notify of all compliance issues.
  • Prepare all reports, as assigned by Director.
