About The Position

This position may be performed on-site, hybrid, or fully remote depending on the selected candidate and organizational needs. The role supports a primarily virtual team. Occasional in-person attendance may be required for training, meetings, or other business needs.

Requirements

  • Understanding of the mission of Prairie Ridge and patients we work with.
  • Minimum of two years’ experience working within the medical office field.
  • Proficiency in managing data collection system and computer management.
  • Proficiency in typing, filing and general office skills, as well as oral and written communication skills.
  • Ability to meet the public and agency patients in a warm, helpful manner.
  • While performing the duties of this job, the employee is regularly required to sit; use hands to finger, handle, or feel objects, tools or controls; and talk or hear.
  • The employee may be required to walk and stoop, kneel, crouch, or crawl.
  • The employee is often required to stand, reach with hands and arms, and climb or balance.
  • The employee may frequently lift and/or move up to 15 pounds.
  • Specific vision abilities required by this job include close vision, distance vision, color vision, and the ability to adjust focus.

Responsibilities

  • Provide appropriate linkages, referrals, coordination, and follow-up for patients to services and supports, including referrals and linkages to long term services and supports.
  • Communicate with internal and external providers to ensure that patient needs for supports and linkages align with basic needs and treatment goals.
  • Provide health care appointment scheduling for patients to mental health, substance abuse, and physical health providers.
  • Communicate with other providers and family members.
  • Work closely with community resources and organizations in coordinating patient services.
  • Advocate for patients who may be vulnerable or need additional support.
  • Connect patients with recovery supports as resources are available.
  • Comply with agency policies, procedures, and regulatory standards.
  • Conduct the Admission process for, Outpatient and prescribing patients, using integrated resources.
  • Help monitor file compliance to ensure completeness.
  • Input patient data from tracking forms as provided by clinicians.
  • Assist clinicians in various duties regarding data completion and retrieval.
  • Provide financial evaluations to patients to establish a sliding fee charge at the time of the placement screening and update this sliding fee charge annually thereafter if the patients is still active. Obtain all appropriate releases to third party payors.
  • Participate with other staff members in patient scheduling.
  • Accept and answer all questions asked by patients or interested parties regarding charges for services provided by Prairie Ridge.
  • Complete a review of admission and financial paperwork at each level of care change and discharge.
  • Maintain all HIPAA and 42 CFR confidentiality requirements and regulations as outlined in CARF policies and procedures.
  • Comply with agency policies, procedures, and regulatory standards.
  • Other duties as assigned by the Support Staff Supervisor or designee.
