About The Position

CAN Community Health is hiring a Patient Access Specialist I who is passionate about patient care and serving individuals impacted by HIV, Hepatitis C, STI's, and other infectious diseases. This role is vital in supporting access to prescribed medications and comprehensive care for patients. The specialist facilitates coordination between prescribers, patients, pharmacy services (including 340B), and financial assistance programs to ensure timely, affordable access to treatment. They function as a liaison to promote medication adherence by providing patient education, resolving insurance or cost-related barriers, assisting with prior authorizations, and supporting prescription renewals and refills. In collaboration with the care team, the Patient Access Specialist also identifies and addresses social determinants of health that may impact access to care or adherence. Additionally, the role supports therapeutic outcomes by monitoring medication access issues, guiding providers through formulary and coverage requirements, and connecting patients to manufacturer copay and patient assistance programs. The ideal candidate is detail-oriented, compassionate, and experienced in navigating pharmacy operations, HIV care systems, and the broader healthcare landscape with a focus on equity and patient-centered advocacy.

Requirements

  • Bachelor’s degree in public health, Healthcare Administration, Social Work, Psychology, Biology, Global Health, or related field and/or three (3) year minimum of relevant work experience
  • Minimum 1-2 years relevant experience (Ryan White, Case Management, Prevention, Pharmacy Technician, etc.)
  • Must be able to operate a motor vehicle and have valid insurance and driver’s license.
  • Must be able to pass a Level I and Level II Background check as required.

Nice To Haves

  • Familiarity with HIV services, Ryan White Program, ADAP, and/or 340B pharmacy preferred.
  • Bilingual in Spanish or Creole.

  • Adhere to and promote compliance with CAN policies and regulatory standards to ensure safe, ethical, and high-quality care delivery.
  • Identify and assist patients in overcoming barriers such as insurance coverage gaps, medication costs, transportation, and social determinants of health (Housing, transportation or food insecurities) affecting medication adherence.
  • Educate patients on HIV care services and CAN-Owned pharmacy options, including 340B benefits, to empower informed, cost-conscious healthcare decisions.
  • Maintain accurate and timely documentation of medication access interventions, follow-ups, and outcomes to support care coordination and program compliance.
