This role is responsible for creating the initial electronic health record, which serves as the foundation for the patient's medical record used by the entire healthcare team. The position prevents the creation of duplicate medical records, which can lead to treatment safety issues and billing problems. It also ensures compliance with accrediting bodies' mandates for using identifiers to positively identify patients before care delivery, ensuring patient safety. The role involves checking in and registering patients, obtaining and verifying complete demographic, guarantor, and insurance information, and collecting co-pays and other out-of-pocket patient responsibilities. Accurate registration information is crucial for timely payment and preventing billing issues and patient complaints. The Patient Access Rep maintains strict confidentiality of patient information in accordance with HIPAA. They possess knowledge of insurance basics, recognize commercial and government plans, understand Advocate Health's contracted plans, and know when a statement of financial responsibility is needed. They can discuss financial information and obligations with patients and refer them to Financial Advocates when necessary. The role requires knowledge of rules, forms, and questions necessary for compliance with government agencies and regulations, such as HIPAA, EMTALA, Consent for Treatment, Patient Rights and Responsibilities, Important Message from Medicare (IMM), Medicare Outpatient Observation Notice (MOON), Notice of Privacy Practices, Medicare Secondary Payer Questionnaire (MSPQ), and Advanced Beneficiary Notice (ABN). Signatures are obtained as required. The position may involve scheduling patient appointments, coordinating cancellations, reschedules, wait list requests, and recall requests. It also provides accurate, detailed information regarding test preparations, patient arrival times, medication/food/beverage guidelines, check-in procedures, and directions. The role fosters a welcoming and professional environment through extraordinary customer service, greeting patients and visitors, responding to routine requests, answering telephones, screening calls, and taking messages. Assistance is offered to patients, including arranging transportation, providing directions, locating wheelchairs, and coordinating interpreter services. The role involves monitoring and working assigned electronic health record work queues according to departmental processes.
