About The Position

Clinica Sierra Vista is seeking a Patient Access Coordinator (PAC) to join their Eligibility and Enrollment Team. The PAC will be responsible for ensuring patients receive adequate support and guidance in obtaining healthcare coverage. This role involves providing program-related enrollment services and conducting community outreach activities. The PAC reports to the Program Supervisor.

Requirements

Graduation from high school or completion of a General Equivalency Degree (GED).
Basic Computer Skills.
Valid California driver license and current automobile insurance.
Precise and conscientious about details.
Excellent communication skills.
Ability to handle multiple tasks well.
Ability to work well independently and with others.
Ability to follow directions, learn new skills and meet deadlines.

Nice To Haves

Bilingual ability (English/Spanish) desirable.
Experience or desire to work with low-income families.

Responsibilities

Assist, support, and screen patients for programs such as Medi-Cal (new and renewals), Covered CA, CSV Sliding Fee Program, Cal-Fresh, Homeless Status, Breast and Cervical Cancer Treatment Program, Every Woman Counts, and Transportation needs.
Schedule and meet one-on-one with patients to assist them in enrolling and obtaining necessary medical care and treatment.
Assist patients with no income or proof of income in completing Self Declaration forms.
Provide superior customer service to patients and team members with an attitude of helpfulness, dignity, and respect.
Present all patients on the Sliding Fee Program with an annual Assurance Card and provide education and information on their healthcare benefits.
Coordinate and/or conduct community outreach activities as directed.
Provide input to the Supervisor regarding site-related issues and suggestions for improvement.
Assist with the collection of data for program and site audits.
Document all notes and encounters in Clinica’s EMR.
Ensure proper follow-up is completed in a timely basis.
Comply with established Clinica Sierra Vista policies.
