About The Position

Pathways HUB Coordinator Full Time/Hybrid/Toledo, OH Health Council of Northwest Ohio (HCNO) is seeking a Pathways HUB Coordinator to provide essential coordination and implementation of HUB programs and grant funded initiatives, playing a key role in ensuring effective delivery of services and positive client outcomes. The Coordinator provides training to and technical assistance to our community partners and supports day to day operations of the HUB including grant management, reporting, and quality improvement efforts. About Us HCNO is a mission-driven, health-focused nonprofit dedicated to improving health outcomes across the 18 counties within Northwest Ohio. HCNO leads innovative programs, community initiatives, and collaborative partnerships that advance population health and strengthen community well-being. The Northwest Ohio Pathways HUB (NWOPH) is a regional community-based care coordination network designed to reduce health and social disparities among at-risk populations. It connects individuals, especially those facing complex medical and social challenges, with community health workers (CHWs) who enroll them in the Pathways Program and help them navigate services and achieve measurable outcomes. The NWOPH serves 18 counties in Northwest Ohio and enrolls approximately 2,500 participants each year, primarily working to prevent infant mortality, maternal morbidity and reduce the impacts of chronic disease.

Requirements

  • A bachelor’s degree or equivalent combination of education and experience in public health, public administration, social sciences, or a health-related field
  • Critical thinker, able to research, design, oversee and complete projects with minimal guidance
  • Proficient in the use of Microsoft Office Suite software
  • Aptitude for project management
  • Excellent written and verbal skills

Nice To Haves

  • Previous experience in public health or community-based organization preferred
  • Previous experience in customer service, training, or supportive environment preferred
  • Entry level understanding of data analysis, project evaluation, and medical terminology helpful
  • Ability to create reports, meet deadlines, and interpret grant language preferred
  • Desire and ability to work with diverse populations; participation in community coalitions desired

Responsibilities

  • Lead or co-lead training around the Pathways Community HUB Model, Pathways database, and other trainings as needed for all Community Health Workers, Care Coordination Agencies, and HUB staff
  • Serve as a grant project coordinator to ensure all reporting and deliverables are met
  • Provide technical assistance to Community Health Workers, Care Coordination Agencies, and staff
  • Assist in quality improvement processes and implementation for HUB operations
  • Serve as a point of contact for Community Health Workers and Care Coordination Agencies regarding important client information, HUB programs, and system issues
  • Audit and complete data integrity of entries by Community Health Workers and HUB staff
  • Create reports and presentations
  • Assist in grant procurement, management and reporting for various funder streams
  • Lead special projects that promote improved outcomes among clients and Care Coordination Agencies
