About The Position

GCBHS has an immediate opening for a PATH Case Worker to serve adults who are experiencing homelessness and connect them to mental health and substance use disorder services in Hamilton County. They are responsible for providing support services to PATH staff and clients. Maintains PATH charts and data. Triages client phone calls, assists with client engagement and housing stabilization, and follows up on client’s success with housing.

Requirements

  • High school diploma or GED required.
  • Previous personal experience with homelessness and/or mental illness.
  • Valid driver’s license, vehicle, and insurable driving record may be required.

Nice To Haves

  • Position may require frequent driving of the employee’s personal vehicle.

Responsibilities

  • Conducts satisfaction surveys including Ohio Outcomes surveys.
  • Follows-up with former PATH clients to confirm maintenance of housing and mental health services and to identify clients who need re-engagement.
  • Assists with client engagement and outreach, including in person visits and calls to last known contacts.
  • Provides on-site assistance with service delivery and coordination.
  • Provides peer support services to PATH clients as needed, including providing connection to Recovery Center.
  • Maintains outreach log.
  • Provides other duties as assigned to support the PATH teams.
  • Attends weekly PATH meeting as available.
  • Performs other related duties as assigned.
