About The Position

The PAT Parent Educator provides personal home visits, child development screenings, and family support services to eligible prenatal mothers and families with young children. Operating under the Evidence-Based Parent as Teachers (PAT) model funded by Department of Health (DOH) – Maternal, Infant and Early Childhood Home Visiting (MIECHV) program, this role focuses on strengthening protective factors, promoting positive parenting, and ensuring school readiness. This position requires strict adherence to model fidelity and precise data tracking for federal performance measures. The role involves conducting regular, structured 60-minute home visits with each assigned family to deliver individualized, developmentally appropriate educational activities using the Parent as Teachers (PAT) curriculum model. It also includes planning and facilitating monthly parent group connections events, managing an active caseload of 20-25 families, and conducting ongoing Home Safety Checklist assessments. Data logging for home visits, family goals, referrals, and follow-ups must be done within 3-5 business days in local (DOH) and PAT database systems. The position also requires tracking federal MIECHV performance benchmarks, demographic data, and participant attrition metrics, as well as administering developmental and behavioral screenings within 90 days of enrollment. Attending Continuous Quality Improvement (CQI) data review meetings and actively recruiting eligible prenatal families and families with children 0-3 years old through community events, clinics, and canvassing are also key responsibilities.

Requirements

  • Child Development Associate (CDA) or an associate degree (AA) in Early Childhood Education or a closely related field.
  • Minimum of one (1) year of professional experience working with at-risk populations, pregnant individuals and families with children 0 to 5 years old.
  • Strict adherence to model fidelity and precise data tracking for federal performance measures.
  • Ability to conduct regular, structured 60-minute home visits.
  • Ability to plan and facilitate monthly parent group connections events.
  • Ability to manage an active caseload of 20 – 25 families.
  • Ability to conduct ongoing Home Safety Checklist assessments.
  • Ability to log data within required local (DOH) and PAT database systems within 3 to 5 business days.
  • Ability to track federal MIECHV performance benchmarks, demographic data, and participant attrition metrics.
  • Ability to administer developmental and behavioral screenings within 90 days of enrollment and according to the child level screening schedule compliance and timelines.
  • Ability to attend Continuous Quality Improvement (CQI) data review meetings.
  • Ability to actively recruit eligible prenatal families and families with children 0-3 years old.
  • Must successfully complete a criminal background check and/or FBI fingerprinting, as well as pre-employment drug screening.
  • Subject to mandatory random drug and alcohol testing.

Nice To Haves

  • Bachelor’s Degree (BA/BS) in Early Childhood Education, Social Work, Human Development, Psychology or related human services field. (Highly Preferred for MIECHV funding compliance)
  • Three (3) or more years of experience in home visiting, case management or working with vulnerable prenatal families and families with children ages 0 to 5 within a structured, evidence-based model.

  • Administers developmental and behavioral screenings within 90 days of enrollment and according to the child level screening schedule compliance and timelines.
  • Attends Continuous Quality Improvement (CQI) data review meetings to analyze program performance measures.
  • Actively recruits eligible prenatal families and families with children 0-3 years old (prioritizing early enrollment per MIECHV guidelines) through community events, clinics, canvassing, etc.
