About The Position

The Program of All-Inclusive Care for the Elderly (PACE) offers a variety of services, with many of them provided onsite at a PACE Center. PACE is an alternative to nursing home care and – through an interdisciplinary care team (IDT) of physicians, nurse practitioners, nurses, social workers, therapists, van drivers, and aides – coordinates and provides preventive, primary, acute, and long-term care services, so older individuals can continue living in the community. Join Senior Care Partners P.A.C.E. as a Participant Care Assistant (PCA) – Recreational Therapy and help enrich the daily lives of our participants through compassionate, person-centered care.

Requirements

  • High school diploma or equivalent
  • At least 1 year of experience working with elderly or long-term care populations
  • Strong communication and interpersonal skills
  • Compassionate, patient, and team-oriented mindset
  • Ability to maintain confidentiality and exercise good judgment
  • Basic computer skills
  • Must have reliable transportation, current valid Michigan driver’s license and car insurance.
  • Must be 18 years of age or older.
  • Must be fully vaccinated, or willing to be fully vaccinated against the following communicable diseases: Influenza, MMR, Varicella, Tdap/Td, Covid-19 and Hepatitis B.

Nice To Haves

  • Experience in geriatrics or dementia care
  • Experience working with cognitively impaired adults

Responsibilities

  • Support and lead therapeutic recreational activities, both group and one-on-one
  • Help design and implement fun, meaningful programs tailored to participant interests and abilities
  • Engage and support individuals with dementia, including memory care programming and sensory activities
  • Assist with community outings and special events
  • Help maintain a welcoming, organized activity space
  • Provide input for monthly activity calendars
  • Assist participants with activities of daily living (ADLs) in the center (and occasionally in the home when needed)
  • Collaborate with families and care teams using a patient- and family-centered approach
  • Document participant interactions and progress in accordance with policy
