The Part Time Hospice and Palliative Nurse Practitioner will provide and document medically necessary services in accordance with physician/NP protocol. This role involves participating in decisions regarding patient recertification and discharge from hospice & palliative care. The NP will visit patients in their homes to assess their appropriateness for hospice care and make treatment recommendations, including writing treatment orders and documenting them. Collaboration with the interdisciplinary care team is essential to ensure client medical needs are met. The NP will develop treatment plans to maximize patient health potential, which includes ordering and interpreting diagnostic tests, prescribing appropriate pharmacologic and non-pharmacologic interventions, developing education plans, and ordering durable medical equipment. Advanced care planning for continuity of care is also a key responsibility. The NP will consult with collaborative physicians, other practitioners, and specialists. Establishing relationships with existing hospice palliative care teams, partner organizations, medical directors, hospital physicians, community physicians, nurses, and community members is crucial for building a collaborative culture. The NP will also work with the PCP to refer patients to specialists or other healthcare services as medically indicated and assist in care coordination for referrals. In conjunction with the Marketing department, the NP will help develop outreach programs and participate in outreach presentations and education. Disease management instruction and education will be provided to patients and their families. Clinical guidance and education will be offered to facility staff regarding patient care issues, assessments, and interventions. Reasonable on-call services will be performed as needed. The NP will participate with the care setting's interdisciplinary team as appropriate. Accurate patient records, charts, and documents must be prepared and maintained for sound medical practice, reimbursement, and appropriate medical uses. The NP will act as a clinical resource for complex cases and safe transitions to other care settings. Compliance with applicable laws and regulations regarding Delegation Protocol Agreements, prescriptive authority, and NP scope of practice is mandatory. The NP will review KAQ policies and services with patients/families and obtain consent for medical care. Collaboration with the Intake department for coverage/payment verification and initiating reimbursement is required. The NP will assist in developing and reviewing clinical practice guidelines/standards to support quality care and associated policies and procedures. The role requires acting with integrity, compassion, and customer focus to ensure quality care. Adherence to employment policies, maintaining confidentiality, and complying with HIPAA regulations are essential. Participation in all required in-service education programs and meetings is expected. Compliance with the dress code policy, including wearing an ID badge and maintaining a professional appearance, is required. Other duties as assigned will also be performed.
