Requirements

  • A passion for supporting families and strengthening communities.
  • Strong communication and relationship-building skills
  • Ability to work independently and manage time effectively
  • Organizational and documentation skills
  • Ability to respond calmly in crisis situations
  • Adaptability in a fast-paced, changing environment
  • Comfort speaking with individuals and small groups
  • Frequent travel for home visits
  • Ability to work in various home and community environments
  • Physical ability to sit, stand, and move between locations
  • Occasional lifting (up to 30 lbs)
  • Ability to remain alert and responsive during visits
  • Participation in required PAT training and ongoing education
  • Bachelor’s degree in a related field
  • Valid driver’s license
  • Ability to pass required background checks

Responsibilities

  • Conduct home visits with families (prenatal through kindergarten)
  • Provide parenting education and child development information
  • Develop individualized plans and set goals with families
  • Use screening tools (ASQ-3, ASQ-SE, Edinburgh, etc.) and make referrals
  • Document visits and maintain accurate records in required systems
  • Coordinate and facilitate group connections (10–12 annually)
  • Build relationships with community partners and service providers
  • Maintain consistent communication and follow-up with families
  • Manage a caseload based on program requirements
