POSITION SUMMARY: The Palliative Care Nurse is responsible for activities and actions related to promoting the services and expertise of hospice and palliative care to the community and to patients and family members. The Palliative nurse is an experienced registered professional nurse who initiates and coordinates the palliative care plan, provides skilled palliative care and coordinates the services provided to his/her patients. The nurse works cooperatively with the attending physician, Palliative Medical Director, and other members of the interdisciplinary Palliative Care team and participates in patient care conferences. The Palliative nurse provides care, as described in this position description, to patients who have been diagnosed with a life limiting illness. The nurse regularly reviews and updates the plan of care, performs prescribed medical treatments, including pain management and symptom control, conducts admissions, assessments and evaluations, provides education and supportive care to patient and caregiver. The Palliative Care Nurse acts as Case Manager for Palliative Care patients and provides supervision and oversight to the Palliative Care LVN.

Requirements

  • Current Professional License in California.
  • At least one or more years of hospice experience.
  • Knowledge of hospice and palliative requirements including Medicare, Medi-Cal, and private insurance guidelines.
  • Ability to communicate effectively orally and in writing.
  • Good marketing skills.
  • Ability to respond to the needs of physicians, discharge planners, patients and family members.
  • Ability to communicate effectively and work well with others maintaining a positive attitude.
  • Valid driver's license, reliable transportation, and auto insurance.

Nice To Haves

  • Bachelor's degree in nursing preferred.
  • CHPN preferred.

Responsibilities
