About The Position

This is a replacement position for a Palliative Registered Practical Nurse (RPN) in a community nursing role. The RPN will conduct scheduled visits within the community to care for patients of all ages with varying complexity of care needs, helping them remain comfortable in their homes. This role offers autonomy, independence, and work-life balance. The position is full-time with a shift from 8 am to 4 pm on weekdays and requires working every third weekend. The role involves on-call coverage, including after hours, to ensure patient safety and meet care needs. The position is part of the UFCW Union Local 175, classified as RPN or LPN as applicable.

Requirements

  • Registration with the CNO as a Registered Practical Nurse in good standing
  • Palliative care experience is mandatory
  • Valid driver’s license and access to a vehicle is required
  • Reliability and flexibility
  • Willingness to travel to and from patient residences within a designated area
  • Experience working with the vulnerable population

Nice To Haves

  • Post-graduate education in Palliative care is strongly preferred
  • Experience in delivering community care is an asset

Responsibilities

  • Participate in scheduled visits in the community to look after patient needs within your scope of practice
  • Provide ethical and safe nursing care
  • Use appropriate assessment tools/techniques to assess patients’ individual needs
  • Execute and continuously monitor care plans and adjust as required
  • Engage patients using a collaborative approach by building rapport and trust
  • Maintain and update professional patient records related to patient encounters and plans
  • Required on-call coverage, including after hours to ensure patient safety and to meet patient care needs
