About The Position
Concordia Visiting Nurses is seeking a compassionate and experienced Palliative Care Nurse Practitioner to join our growing team! This role offers the opportunity to make a meaningful impact by providing patient-centered care to individuals facing serious illness in the comfort of their homes and care facilities.
Requirements
- A compassionate and patient-focused Nurse Practitioner
- Passionate about improving quality of life for patients with serious illnesses
- Experienced in palliative care, hospice, home health, geriatrics, or chronic disease management
- A strong communicator who values teamwork and patient advocacy
- Licensed and board certified as a Nurse Practitioner in Pennsylvania
Responsibilities
- Provide comprehensive palliative care assessments and symptom management
- Support patients and families through goals-of-care discussions and advanced care planning
- Collaborate with physicians, nurses, social workers, and interdisciplinary team members
- Manage complex and chronic conditions with a focus on comfort and quality of life
- Educate patients, caregivers, and staff on palliative care services
