About The Position

The Nurse Practitioner is responsible for the provision of preventive medical, palliative, and educational services to promote patient health and wellness and uses standard software and the comprehensive clinical-financial computer system ensuring compliance in accordance with company standards and federal, state, and local standards, guidelines, and regulations. Requirements

  • Current unencumbered license for the state of Ohio to practice as a Certified Nurse Practitioner (CNP) required.
  • Current Certificate of Prescriptive Authority or Externship Certificate to Prescribe.
  • Two years experience in the home health and/or hospice field required.
  • Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint), and the internet required.
  • Must be able to read, write, speak, and understand the English language.
  • Must possess a valid driver's license and acceptable driving record.
  • Must be insurable under professional liability and crime coverage policies as specified by insurance carrier underwriting standards.

Nice To Haves

  • Proficiency with electronic medical records (EMR) preferred.

Responsibilities

  • Conducts comprehensive medical and social histories and performs physical examination of healthy, acutely ill, chronically ill patients or terminal patients.
  • Treats common acute illnesses, chronic and stable health conditions through prescribing medications and other treatments.
  • Orders and interprets lab tests and x-rays.
  • Provides information, instruction, and counseling on health maintenance, health promotion, social problems, illness prevention, illness management, palliative care and medication use.
  • Refers patients to other health professionals as needed.
  • Communicates with staff members, patients and families to promote optimal patient outcomes.
  • Completes documentation accurately and timely while maintaining state, federal and agency regulation.
  • Participates in quality improvement projects.
  • Commitment to excellent customer service for internal and external customers.
  • All other duties as assigned.
