About The Position

The Palliative Care Social Worker serves as the program’s primary liaison, connector, and system navigator across the full continuum of care. This role bridges all care locations to ensure patients and families experience seamless, compassionate, and coordinated care. The PC-SW plays a pivotal role in driving program growth, improving timeliness and appropriateness of referrals, and supporting continuity of care from inpatient to community settings. By proactively identifying eligible patients, facilitating goals-of-care conversations, and managing psychosocial complexity, this role frees significant provider time—allowing MDs and NPs to expand visit capacity and increase census.

Requirements

  • Bachelor’s degree in Social Work from an accredited program.
  • Minimum 2–3 years of experience in medical social work, palliative care, hospice, oncology, behavioral health, or other complex care settings.
  • Strong competency in psychosocial assessment, crisis intervention, navigation of community resources, and complex care coordination.
  • Ability to travel as required.

Nice To Haves

  • Prior experience in hospital systems, emergency departments, or multi-site healthcare environments.
  • Master’s degree in related field.
  • Knowledge of local community resources, long-term care, home health, and hospice systems.
  • Bilingual skills (as applicable to population served).
