About The Position

The Palliative Care Resource Liaison serves as a hospital-based advocate and educational resource for patients, families and medical care team members to facilitate best practices for end-of-life and chronic care services.

Requirements

• Bachelor's in Nursing OR Masters in Social Work or related field.
• Critical thinking skills
• Effective communicator with difficult conversations about goals of care
• Experience with end of life or chronic care services
• Ability to develop positive relationships
• 1-2 years working with hospice and/or palliative care patients

Nice To Haves

• Licensed Clinical Social Worker
• Advance Care Planning Certified
• Hospice and Palliative Nurse (CHPN)
• 5+ years working with hospice and/or palliative care patients

Responsibilities

• Collaborates with medical team members to identify needs and advocates for patients and families needing palliative care or hospice services.
• Supports discussions about patient and family concerns and goals of care to include health care surrogacy, advanced care planning, and symptom management.
