About The Position
Preferred Care Home Health is seeking a compassionate, experienced Palliative Care RN to join our interdisciplinary team serving patients and families throughout Lee County. In this role, you'll act as Case Manager delivering comprehensive, patient-centered nursing care in the comfort of patients' homes and communities.
Requirements
- Active Florida RN license
- Graduate of an NLN-accredited nursing program
- Minimum 2 years of nursing experience, including at least 1 year in home care, palliative care, or hospice
- Current CPR certification
- Valid driver's license, reliable insured vehicle
- Strong communication, critical thinking, and organizational skills
- Ability to work independently with minimal supervision
- Physical ability to walk, stand, bend, and assist with patient transfers
Nice To Haves
- Palliative care/hospice training or certification preferred (or eligibility for certification)
Responsibilities
- Conduct comprehensive initial and ongoing patient/family assessments
- Develop and update individualized care plans in collaboration with the interdisciplinary team
- Administer medications and treatments as ordered
- Educate patients and families on advance care planning, disease management, and self-care
- Coordinate care with physicians, community resources, and team members
- Supervise home care aides and support ancillary staff
- Participate in on-call rotation and interdisciplinary team meetings
