About The Position

Provides family centered palliative care to patients and their families who are facing life limiting and/or life threatening illness. Care is provided in the inpatient and outpatient setting. Requirements

• Bachelor's Degree
• 3-5 years experience
• Pediatric inpatient experience or hospice/palliative care experience.
• Registered Nurse Multistate License Missouri OR Registered Nurse Multistate License Kansas OR RN Compact license for all states besides KS and MO required upon hire
• Required Life Support Certifications Required Upon Hire
• Specialty certification (CHPPN) within two years of hire

Responsibilities

• Conducts a comprehensive assessment of patients and their families who are experiencing life-limiting or life-threatening illness, then leading/participating in conversations that implement and discuss palliative care concepts.
• Communicates effectively with patients, families, and healthcare professionals.
• Promotes palliative care concepts and an interdisciplinary team approach in the provision of palliative care to the patient with serious or life threatening illness and their family.
• Achieves and maintains expertise in Palliative Care concepts and demonstrates knowledge, skills, and confidence when providing palliative care education to patients, families, healthcare professionals and the community. Participates in ongoing development of individual palliative care staff and the process by which palliative care is delivered at CMH.
